To make the Fertility Assessment process seamless, we collect a $450 non-refundable deposit upfront. This deposit is applied as a credit to your account and can be used toward your consultation, follow-up visit, or any out-of-pocket costs, such as co-insurance or deductible. If your insurance covers some or even all costs, you can apply the credit that remains to future services with us.
Our modern software guides you (and, if you have one, your partner, at no additional cost) step by step through the process of completing your intake paperwork and sharing your medical history. You can self-schedule your consultations and diagnostic testing appointments, and live chat with your Onto care team.
We’ll assess your fertility through blood work, scans, genetic testing and a semen analysis, as indicated by your Onto Clinician. Scheduling and orders for your diagnostics are included but the costs of the diagnostics themselves are not. If desired, we’ll help you run these costs through your insurance.
When your diagnostic results are ready your Onto Clinician will prepare an intuitive Fertility Assessment Report. No cryptic lab paperwork here; the information we share with you is easy to understand and digest. We’ll share results for each diagnostic element paired with a clear explanation of what it means for you.
Receive personalized guidance from a fertility specialist via included telehealth consultations, both at the start of your Fertility Assessment and then after your diagnostics results are in, to discuss the best treatment options for you. It’s your decision whether to proceed with treatment now, or at any time in the future.
The Fertility Assessment includes services that may be billed to insurance; however, we require a $450 non-refundable deposit to secure your spot. This deposit is applied as a credit to your account and can be used toward any patient responsibility, including co-insurance, deductible, or services not covered by your insurance.
Diagnostic testing and imaging related to fertility care may be covered by your insurance; however, coverage depends on your specific insurance plan and does not necessarily mean the services will be free.
Speak with our team at no cost to you to get started. If you desire, before we speak with you we can verify whether your insurance is active, confirm if it is in-network with us, and determine what your benefits might be for your Onto Fertility Assessment.

During your initial consultation, we’ll provide a personalized timeline based on your medical history, fertility goals, and the treatment you choose. Yes, pregnancy is possible after tubal ligation, either through tubal reversal surgery or in vitro fertilization (IVF). IVF is often the preferred method because it bypasses the fallopian tubes entirely, making it a great option for individuals who have undergone tubal ligation.
