The Fertility Assessment includes services that may be billed to insurance; however, we require a $450 non-refundable deposit to secure your spot. This deposit is applied as a credit to your account and can be used toward any patient responsibility, including co-insurance, deductible, or services not covered by your insurance.
Diagnostic testing and imaging related to fertility care may be covered by your insurance; however, coverage depends on your specific insurance plan and does not necessarily mean the services will be free.
If you choose to provide your insurance information we will verify whether your insurance is active, confirm if it is in-network with us, and determine what your benefits might be for your Onto Fertility Assessment. We’ll share our findings with you when we speak on your Complimentary Navigation Call.
