Legal Disclaimer for Onto Fertility’s “Treat Yourself Giveaway”

1. Eligibility:
   The “Treat Yourself Giveaway” is open to legal residents of the United States residing in Colorado, who are 18 years of age or older at the time of entry. Employees of Onto Fertility, its affiliates, subsidiaries, advertising and promotion agencies, and their immediate family members are not eligible to participate.
2. No Purchase Necessary:
   No purchase is required to enter or win. Making a purchase does not increase your chances of winning.
3. Entry Period:
   The giveaway begins on January 14, 2025, at 12:00 AM MST and ends on February 13, 2025, at 11:59 PM MST (“Entry Period”). Entries submitted before or after the Entry Period will not be eligible.
4. How to Enter:
   To enter, participants must subscribe to the Onto Fertility email newsletter by completing the subscription form at https://onto.health/treat-yourself-giveaway-win-a-500-spa-day during the Entry Period. Limit one (1) entry per person. Duplicate entries will be disqualified.
5. Prize Description:
   One (1) winner will receive a $500 gift voucher for a spa day. The prize is non-transferable and cannot be redeemed for cash or substituted, except at the sole discretion of Onto Fertility.
6. Winner Selection and Notification:
   The winner will be selected in a random drawing conducted by Onto Fertility on or about February 14, 2025. The winner will be notified via the email address provided at the time of entry. If the winner does not respond within 48 hours of notification, Onto Fertility reserves the right to select an alternate winner.
7. Odds of Winning:
   The odds of winning depend on the total number of eligible entries received during the Entry Period.
8. Taxes:
   The winner is solely responsible for any applicable federal, state, and local taxes associated with the acceptance and use of the prize.
9. Consent to Use:
   By entering, participants grant Onto Fertility the right to use their name, likeness, and entry for advertising, promotional, and other purposes in any media without additional compensation, unless prohibited by law.
10. Release and Liability:
    By entering, participants agree to release and hold harmless Onto Fertility, its affiliates, officers, directors, employees, and agents from any claims or liability arising out of participation in the giveaway or acceptance/use of the prize.
11. Privacy Policy:
    Personal information collected in connection with the giveaway will be used solely for administering the giveaway and for marketing communications from Onto Fertility, in accordance with Onto Fertility’s Privacy Policy, available at https://onto.health/privacy-policy.
12. Void Where Prohibited:
    The giveaway is void where prohibited or restricted by law.
13. Sponsor Information:
    The giveaway is sponsored by Onto Fertility, located at 12770 Lynnfield Dr, Parker, CO 80134.
14. Modification and Cancellation:
    Onto Fertility reserves the right to modify, suspend, or cancel the giveaway if circumstances arise that compromise its fairness or integrity.
15. Dispute Resolution:
    By entering, participants agree that any disputes, claims, or causes of action arising out of or in connection with this giveaway shall be governed by the laws of the State of Colorado and resolved exclusively in the courts located in Douglas County, Colorado.
